Many women conceive quickly the first time but struggle the second time. They wonder why it is not happening now when it happened immediately before. Dr Rashmi Nifadkar, a fertility specialist at Nova IVF Fertility in Baner, Pune, has answered this question.
Dr Nifadkar explained that there is often a gap of a few years between the first and second child, and the woman grows older in that time. With age, the number and quality of eggs begin to decline, so conceiving can take longer than before.
An example makes this clearer. A woman who conceived easily at 27 may have difficulty conceiving again a few years later, at 34 or 36, for instance. Conceiving easily the first time does not mean the second time will be the same.
A woman may put on weight after her first child. Thyroid problems, diabetes, PCOS or other hormonal problems can also develop. These can make menstrual cycles irregular and affect ovulation, which can also delay conception.
It is not only the woman's health that matters. The man's health also has an effect. Advancing age, weight gain, smoking, alcohol, stress or certain illnesses can reduce sperm count and quality. If conception is not happening, it is therefore important for both the woman and the man to be tested.
When there is difficulty the second time, the history of the first pregnancy and delivery is also reviewed. Whether there were complications during delivery, and whether any illness or surgery followed, is also important information.
As needed, doctors can check whether menstrual cycles are regular, whether ovulation is happening, how much egg reserve there is, and the condition of the uterus and fallopian tubes. The quality of the man's semen is also examined. A fertility check-up does not mean IVF will have to be done immediately. The tests are done to find out exactly where the problem lies.
If conception has not happened despite trying for a long time, if periods are irregular or if the woman is older, it is important to consult a specialist at the right time. That conceiving a second time is taking a while does not mean conception will not happen.
